Antenatal care record

The antenatal record captures structured data across all pregnancy visits.


BOOKING VISIT

LMP, EDD (by dates — Naegele rule), EDD confirmed by scan (CRL-based)

Blood group and antibody screen

Booking bloods: FBC, rubella IgG, VDRL, HIV, HBsAg, HCV

BMI, booking BP, urine protein dip

Past obstetric history (gravida, para, previous CS, miscarriage)

Medical and family history

Pre-eclampsia risk stratification (NICE criteria: ≥1 high-risk or ≥2 moderate-risk → aspirin 150mg from 12 weeks)

GDM risk screening (WHO 2013 criteria → OGTT at 24–28 weeks)


EACH SUBSEQUENT VISIT

Gestational age (by dates and scan), next visit date

Fundal height (cm), fetal lie and presentation

Fetal heart rate (FHR bpm)

BP, urine protein dip, oedema grade (0–3+)

Symptom check: PV loss, contractions, fetal movement


SCAN INTEGRATION

Dating scan (CRL, EDD confirmed, chorionicity for twins)

NT scan (11–13+6 weeks — NT measurement, nasal bone)

Anomaly scan (18–22 weeks — structured findings by system)

Growth scan (28, 32, 36 weeks for at-risk pregnancies)

Each scan report is uploaded as PDF or entered structured; EDD auto-updates from CRL.


DELIVERY RECORD

Mode of delivery, indication for CS if applicable

Complications, blood loss, Apgar scores, birth weight

Cord blood sample, vitamin K given, eye prophylaxis

Neonatal outcome linked to maternal record

IVF and IUI cycle tracking

The fertility module tracks complete ART cycles.


IUI CYCLE

Stimulation phase: daily FSH dose, follicle count and size (each ovary), E2 level

Trigger day: hCG or GnRH agonist, follicle size at trigger

IUI day: sperm parameters (count, motility, morphology post-wash), insemination time

Luteal support: progesterone type, dose, start date

Outcome: serum hCG at day 14, clinical pregnancy at 6 weeks


IVF / ICSI CYCLE

Stimulation: daily gonadotrophin dose and brand, E2/LH/P4 levels per day, serial follicle counts

Trigger: criteria met (≥3 follicles ≥17mm, E2 level), trigger type and time

Retrieval: total oocytes, MII oocytes, degenerated, ICSI vs conventional IVF

Fertilisation: 2PN at day 1

Embryo development: day 3 cell count and grade, day 5/6 blastocyst grade (Gardner grading: ICM A/B/C, TE A/B/C)

Transfer: date, number of embryos transferred, blastocyst vs cleavage, fresh vs frozen

Luteal support: detailed regimen

Outcome: βhCG at day 12, gestational sac at 5 weeks, heartbeat at 6–7 weeks


Hormonal trend chart: E2, LH, P4 plotted across the stimulation phase — the view RE clinicians use most.

Cumulative cycle history: all cycles in a patient timeline with outcome and key parameters per cycle.
