SCORE2, GRACE, and CHA₂DS₂-VASc: Why Risk Calculators Belong in Your EMR

Cardiovascular risk scores are only useful when they're computed consistently at every visit. Clinit embeds SCORE2, GRACE, CHA₂DS₂-VASc, and six other validated scores into the cardiology workflow.

The Consistency Problem

Every cardiologist knows these scores. Few compute them consistently at every relevant visit. The barrier is not knowledge — it's friction. Opening a separate calculator app, manually entering the same demographics already in the chart, and then manually recording the result somewhere in the notes is a workflow no one sustains under pressure.

Scores Embedded in Clinit Cardiology

Each calculation pulls demographics and vitals already in the chart. The clinician confirms the result and adds a narrative; AI generates a plain-language ESC-aligned summary.

GDMT Tracker

For heart failure patients, the Guideline-Directed Medical Therapy tracker shows at a glance whether ACEi/ARB/ARNI, beta-blocker, MRA, and SGLT2 inhibitor are prescribed at target doses. Missing or sub-target medications are highlighted.

Device Registry

Pacemakers, ICDs, CRT-D, and loop recorders are registered with device type, manufacturer, model, serial number, and implant date. The registry sends alerts 3 months before warranty expiry and tracks remote monitoring follow-up dates.
